How to Take the Survey at SurveySDM.com
1. Save your receipt
You’ll need the 15-digit Certificate Number printed on your Shoppers receipt. Make sure it’s from a visit within the last 7 days—older receipts won’t work.
2. Visit SurveySDM.com
Choose English or French, then enter the Certificate Number to begin the survey.
3. Share your feedback
I was asked about:
- How my visit went
- Whether I found what I was looking for
- How helpful the staff were
- My checkout experience
It only took me about 3–5 minutes to complete everything.
4. Enter the sweepstakes
Once you’re done, you’ll be asked for your name and contact info so you can be notified if you win. That’s it!
