What is the Domino’s Survey?
The Domino’s Customer Satisfaction survey is Domino’s way to learn what their customers would like to see change about their food, stores, customer service, deliveries, etc… It is also how they find out what their customers already enjoy about their experiences with Domino’s! They will use this information to better serve you and others in the future!
How to enter:
You can enter one of 2 ways: the first is by completing the online survey and the second is to enter by mail!
Enter online:
To enter online by completing the Customer Satisfaction Survey you will need to go to www.DominosFeedback.com where you will be asked to answer questions, rate your experience and provide your current contact information (in case you are drawn as a winner!)
Enter by mail:
To enter without completing a Survey, print your name, address, phone number, email address, and date of birth on a piece of paper and mail it in an envelope with proper postage to “Domino’s Voice of the Customer Survey Sweepstakes” c/o Merkle Inc., P.O. Box 5022, Department 860981, Kalamazoo, MI 49003-5022. You will receive one entry into the drawing.
Rules:
To be eligible to enter the sweepstakes you must be 18 years of age or older, and you must be a legal citizen of the United States. You will NOT be eligible if you are an employee of Domino’s, if you are an immediate relative of an employee and/or if you are a member of the same household as an employee.
Prizes:
There will be a total of 600 Grand Prizes during the sweepstakes, 100 prizes will be drawn each month: Each winner will receive one $25 Domino’s e-gift card. Prizes will be fulfilled approximately 8-10 weeks.
Limit:
You may enter once per month. There is a limit to one entry per email and/or household.
Drawings:
The drawings will be held the second week of each month, any winners will be notified by email, phone or certified mail. Please be sure to provide accurate contact information. Failure to respond within 14 days of the drawing will cause you to lose your spot as a winner and a new name will be drawn in your place.
Length of the survey:
The survey will take under 5 minutes to complete! It is very simple and straightforward!
